Please use this form to request funds from the Cadiz Rotary Club for your Organization! A written request must be received by the Youth & Senior Services Committee for Approval. You may use this form to print out if you want to receive the request form for your own use or your Organization’s use.
· ELIGIBLE ORGANIZATIONS: Any institution or organization in Trigg County that provides direct services to the youth and senior citizens of Trigg County.
· REQUEST PROCEDURE: The representative or entity requesting the funding shall submit directly to the club president which also serves as the chairperson of the Youth and Senior Service Committee, seven (7) copies of a signed and dated Funding Request Form along with other information to justify and/or explain the need for Rotary funding. Or, you can use this form to request funds.
· PRESENTATION OF REQUEST: The treasurer will issue a check within five (5) working days of committee action, if the request is approved.
CHECK PROCEDURE: Checks are never payable to an individual(s). Checks must always be payable to the requesting entity with a notation of the purpose of the funding as a memo in the left hand corner of the check.
